Some just do crude FFT and pulse analysis.

Some do pattern recognition.

Some do really deep level statistical analysis against a database of known, tested and medically tracked profiles (firstbeat app in S5 only does this).

Most are inaccurate, misleading and as such crap shoot.

The gold standard are stuff from Polar, Suunto, HeartMath and FirstBeat.
